How to "Refine Edges"

Hi,

How do you get the Refine Edges tool to work? I make all the changes I want and it shows up in the preview, but hit OK and they disappear!

I must be doing something wrong, because I can’t get any ‘selection box’ changes, like feathering or smooth, to work on my image whether or not I use the Refine Edges dialog. AND I’ve tried it in CS3 and my old Photoshop7, neither one will modify the image! How do you get this to work? It looks fine in the preview, hit OK, and the image remains the same. Anyone know what I am doing wrong?
