I just encountered this problem with Photoshop 5.5. It just started happening for no reason. I reinstalled, reset the .ini file and the save for web preferences. Then I had a thought that it might be the image itself.
I made a screen capture of it at 100%, cropped and resaved, and lo and behold, it saved for web without a problem that time.
Go figure. These were digital images from a digital camera. Maybe some images are copy protected in some way.
