Levels Command unavailable

In PS 6.01, reinstalled after disk crash, now the Levels command is not available (on tool bar nor with kbd shortcut CTRL L). When executed, I get the color picker on the tool bar, and all other commands are greyed out. Other ADJUST commands work Okay. Any suggestions?

Dialog box is offscreen.
You can hit Enter or Escape to continue.

Used higher screen rez recently? Or two monitors?

Increase screen rez to highest, see if you see dialoge.

If not, delete prefs.
