Hello:

I recently developed a webpage in Photoshop CS and Imageready (Adobe). I wish to include a text field in the page (form – input), can some one pls help me with documentation/tutorials on how to accomplish this?

Do I need to do this in Photoshop/Imageready or Dreamweaver?

This isn’t generally the sort of thing you would use an image editor for.

You have a couple of choices–either open the HTML file in an HTML editor, such as GoLive or Netscape Composer, or open the HTML file in a plain text editor like Notepad and edit the HTML yourself.
