I have an action that will work fine then in a few days/weeks it will skip a step (levels) and always the same one. What might be the cause and is there a way to adjust/delete/insert a step without having to re-record the whole thing over again?

Is that step unchecked in the Actions palette?

Is the file it fails on in Indexed Color mode? Can't do a Levels adjustment on those puppies.

Yes, to delete a step, select (highlight) it and then open the Actions flyout menu by clicking on the black triangle on its right side of the palette. Click delete and confirm.
To add a step to an existing Action, select the step above which you want the new step to appear, and then click Record. Add as many steps as you like, and click Stop.
