JPEG Repair in Image Doctor. <http://www.alienskin.com/idoc/idoc_filters.html> It’s the only tool you’ll ever need. I don’t know of any manual way to get rid of them using only Photoshop.

try Neat Image. Its free and outstanding and in my view better than alien skin. But best depends on your definition of best and whatever you use you will have to learn. there is no instant answer. You might want to read the following,which is pretty comprehensive

Shoot, I know there was this one great tutorial on this site a while back, but I can’t find it now! It involved creating an edge mask, converting the file to Lab mode, median and Gaussian Blur on the A and B channels, then using the edge mask to preserve detail run a tiny amount of Smart blur.

Actually, Mathias has an action which is similar to this process:

I see this link to the ADOBE site on many pages, but when I search the site I cannot find the tutorial. Anybody remember it ?, or CAN find it ?
