Could anyone tell me how I can combine all the layers in a Photoshop document into a single new
layer without deleting the originals.

Save a copy of your file. Then you have your original, and one you can flatten. Sometimes the answer is too easy :-)

Maintain the ALT key while you choose merge visible layers in the "arrow" menu
after of course add a new and empty layer.

To create a new layer that is a combined version of all layers hold down Ctrl+Al;t+Shift (Mac:Cammand+Option+Shift) and type N E
That will do the job. Good luck.

You could link all your layers together, and then click on the tiny triangle at the top right corner of the layers palette and create a New [Layer] Set from Linked. This will put all the layers into a single folder. Then you could click on the tiny triangle again and Duplicate Layer Set. Inside this Layer Set, the layers will all be linked as in the original. Click on one layer and press CTRL+E to flatten them all into a single layer. This folder now contains the single (merged) layer, while the other contains all the individual layers.
