This is not a nearly finalized version, NOT EVEN CLOSE, but just to give you an idea of what you could start playing with.
< http://www.pixentral.com/show.php?picture=1YzffkxN6d83SBYFcX UDinF2nPS> Smoke created by new layer, paintbrush draw smoke, gaus. blur, then a little liquify filter for shape, and finally lowering the opacity of the layer itself. To get a more "real" smoke you’d probably want several layers at different opacities etc. Just threw in the heart as silly shape idea. Lit cigar, just add some red an black, blur edges until it looks "right".
Mostly with things like this you can go through several versions until you find something finished enough that you like.
