Create a template to fill in a "form" to use with word processor

Is it possible to:
1. scan a document (a blank printed "form")
2. make a appear as background in a word doc; like stationary
3. enter text into the document, using the background (form) as a
guide to get the text in the right fields; as it were
4. print the page, without the background, directly onto the actual, pre-printed, form that has been placed in the printer?

I would look at Adobe Acrobat as a software package for form filling.

It should be possible after defining the fill fields to bleach out the scanned image so it does not print.
