How can I verify that my custom monitor profile is actually being used by Photoshop 7.0.1, running on Mac OS 10.3.3?

In your Color Settings, choose "Color Management Off". The monitor profile will show up as the RGB working space. Be sure to hit Cancel when you’re done.

Or just look at the RGB Working Space menu in Color Settings — it will contain an entry labelled Monitor RGB. The profile name appears after the dash following Monitor RGB.

REALLY! UH OH! Mine says Generic RGB Profile! OMG! How do I get it to use my calibrated profile???
