You are probably using the ‘recent files’ feature of Photoshop. Try using File Open and see if there is a different version of the file on your drive.

Also, Photoshop ‘save for web’ and ImageReady do not automatically save the optimized file in the same directory the original was in. Both automatically assume you want to save it in the folder you last saved a ‘save for web’ or ImageReady file in. This is NOT necessarily the folder that the original file was in. Finally, the optimized file will have dashes and underlines in the name so that will be a clue for you to identify the file.

When a file is open in PhotoShop, it reports the uncompressed size, pretty much the same as if it were a TIFF. This always differs from the size on disk of the file in the case of JPEGs.
