You’ll need two mujahideen, a howitzer and a camera 😉
Seriously, IMHO this is not a PS effect but rather a snapshot taken through a muzzle.

Yes, there might have been extra hardware invloved, but I’m suprised no one has any ideas on how the effect can be achieved in Photoshop.

Create a layer go to filter/distort/twirl. You’ll need to use this layer with others and masks to create the image.
