Tricky problem – how do I angle this in Photoshop?

If I’ve got a screen shot, or a box, whatever – how do I do this with it: http://quarterbacks.org/images/frontscreen-angled.gif

A screen shot/box, is angled the right side away, and made it look farther – see, I can’t even DESCRIBE it so I can’t even Google for how to do this…!

I’ve got Photoshop 7.

Edit – Transform – Perspective
Next, you might have to fix the width so apply Edit – Transform – Scale.

Yes. Looks like they missed that last step in the example. It looks "stretched".
