Image Size Confusion

When I try to size an image to specific dimensions (for example, 4 inch width and 4 inch height), PS always re adjusts the size– I might end up with an image that is 4 inches by 4.2 inches. I’ve tried doing this with Constrain Proportions & Resample checked off, but it doesn’t seem to matter. What am I missing here?

If you have resample unchecked then constrain proportions will be on and you can not turn it off. you have to turn constrain proportions off to get the size you want. also photoshop does not work by inches it works by pixels, so if you tell it 4 inches that may not work out to an exact number of pixels and ps will have to go to an exact pixel, this amount will vary depending on your ppi.
